• Herb refill requests can only be completed for current patients of Active Life.

• If you haven't been in to the clinic for an herb consult in awhile, we may request that you book a consultation before we refill your herbs, so that we can be sure we're delivering the safest and most effective care.

• We require that you have a credit card on file to refill formulas. If you don't currently have a card on file, please call us at 479-358-7690. All herbal formulas are custom made for the individual and cannot be returned to our inventory after they have been made.
